Partner Program Terms

1. The program
The Intake Form Pro Partner Program lets approved affiliates refer prospective customers to Intake Form Pro using a personal tracking link. Partners are independent affiliates and are not employees, agents, or legal representatives of Intake Form Pro. Participation requires approval, which is granted at our sole discretion and may be declined or revoked.
2. Commission
Approved partners earn a commission of 40% of the net amount actually paid by each referred customer on every subscription invoice, for as long as that customer remains an active, paying subscriber. Commission is credited per paid invoice. There is no monthly cap and no fixed expiry — commission continues for the life of the referred subscription.
3. Attribution
Attribution is first-touch: the first partner tracking code recorded for a visitor is the one credited, and it is never overwritten by a later code. The tracking cookie lasts 90 days. A referral is credited only when the referred customer creates an account while attributed to the partner and subsequently pays.
4. Churn and reversals
If a referred customer cancels their subscription, no further commission accrues for that customer. Past, validly earned commissions are not clawed back. Commissions associated with refunded, charged-back, or fraudulent payments may be reversed.
5. Payouts
Partners may request a payout once their pending balance reaches the minimum of $50.00. Payouts are made via the method on file (manual, PayPal, or bank transfer) and are marked paid once processed. Partners are responsible for providing accurate payment details and for any taxes owed on commissions earned.
6. Acceptable conduct
Partners must promote Intake Form Pro honestly and lawfully. Spam, misleading claims, trademark misuse, self-referrals, cookie stuffing, bidding on our brand terms, or any deceptive practice is prohibited and may result in withheld commissions and termination from the program.
7. Changes and termination
We may modify these terms or the commission structure on a going-forward basis, and may suspend or terminate any partner for violation of these terms. Either party may end participation at any time; validly earned, unpaid commissions above the minimum remain payable.
